When you contact Baseline Recovery, arrange an appointment or receive treatment, I may collect personal information needed to communicate with you, provide treatment safely and maintain appropriate client records.
This may include your name, date of birth and contact details; relevant medical and health information; injuries, symptoms and areas of concern; medication and medical history; lifestyle, occupation and activity information where relevant; consultation and assessment information; treatment provided, outcomes and progress; aftercare advice; appointment and communication records; and consent information.
Some of the information collected during your consultation is health information and is treated as sensitive personal information under UK data protection law.
Your information may be used to respond to enquiries and arrange appointments; assess whether treatment is appropriate for you; provide treatment safely and effectively; maintain accurate consultation and treatment records; monitor your progress; provide appropriate aftercare; and communicate with you about your appointments or treatment.
I may also need to retain and use information to meet professional, insurance and legal requirements, or where necessary in connection with a complaint or legal claim.
Because Baseline Recovery provides sports massage therapy, I may need to collect information about your health, medical history, injuries, medication and other health-related matters where relevant to your treatment.
This information helps me assess and provide treatment safely and appropriately. Please let me know if relevant aspects of your health or medication change during the course of your treatment.
Appropriate records are kept for each treatment. These may include details of your consultation and assessment, treatment provided, the outcome of treatment and any aftercare advice given.
I take appropriate steps to keep your personal information secure and protect it against unauthorised access, loss, misuse or disclosure.
Digital client records are stored using password-protected systems. Where records are kept in paper form, they are stored securely in a locked location.
Access to client information is restricted and information will only be shared where there is an appropriate reason for doing so.
Client consultation and treatment records are retained in accordance with my professional insurance requirements.
For adult clients, treatment records are kept for at least 7 years following the last treatment.
Where treatment is provided to someone under the age of 18, records may be retained until at least their 25th birthday.
Information may be retained for longer where there is a legitimate legal, insurance or regulatory reason to do so. Other personal information will only be kept for as long as reasonably necessary for the purpose for which it was collected.
Your personal information is treated confidentially and will not ordinarily be shared with third parties.
Information may be shared where you have asked or authorised me to do so; where it is necessary to communicate appropriately with another healthcare professional; where disclosure is required by law; or where necessary in connection with an insurance matter, complaint or legal claim.
I may also use trusted service providers, such as website, email or booking providers, where they process information on my behalf. Where this happens, reasonable steps are taken to ensure your information is handled appropriately and securely.
If you contact Baseline Recovery through the website, by email or through another contact method, I may retain the information you provide in order to respond to your enquiry and, where appropriate, arrange an appointment or provide services.
Please be aware that ordinary email is not necessarily a completely secure method of communication. You should avoid sending unnecessary sensitive medical information by email. Relevant health information can instead be collected as part of your client consultation.
The Baseline Recovery website may use cookies and similar technologies to enable the website to function and, where applicable, understand how visitors use it. Where treatment is provided to someone under the age of 18, personal and health information may be collected where necessary to provide treatment safely and maintain appropriate records.
Appropriate consent will be obtained where required. For clients under the age of 16, parental or guardian consent will normally be required, subject to circumstances where the young person is able to provide valid consent themselves.
Treatment records relating to clients under 18 may be retained until at least their 25th birthday, in accordance with my professional insurance record-keeping requirements.
